A. Persyaratan Pengajuan Ijin Usaha Industri (IUI)
- Persetujuan prinsip [bagi perusahaan yang dipersyaratkan] (IB & IM);
- AMDAL bagi yang diwajibkan (IB & IM);
- UKL/UPL bagi yang tidak wajib AMDAL;
- SPPL (bagi IKRT);
- Surat keterangan domisili perusahaan dari desa/kelurahan.
- Ijin HO (bagi yang dipersyaratkan);
- Foto copy Akte pendirian perusahaan dan menunjukkan aslinya;
- Foto copy KTP pemilik/penanggungjawab dan menunjukkan aslinya;
- Foto terbaru ukuran 4 X 6 = 2 lembar;
